A reviewer's time is scarce. Do the work that lets them focus on judgement, not on reconstructing what you did or why.
What to provide
- Context. One or two sentences on the problem and why this change solves it. Link the issue or the failing behaviour it addresses.

- Risks and areas to scrutinize. Point the reviewer at the parts you are least sure about.
Rules
- Keep the change focused. A diff that does one thing reviews faster and merges safer than one that also cleans up three unrelated things.
- Review your own diff first, as if you were the reviewer. Fix the things you would flag before asking someone else to.
- Make the description match the diff. If they disagree, the description is a lie the reviewer will trust and be misled by.
